The University requires a printed and signed Medical Clearance Form, and a copy of the front and back of your Medical Insurance Card, before your camper is allowed to participate.  Please print and prepare these requirements.  These forms may be included during online registration, or, if you have already registered use the Forms submission below to attach forms to your registration.
